How to Report an Incident or Concern
If you have a concern involving campus safety, academic misconduct, student conduct,
discrimination, or a student who may need additional support, use the reporting options
below. Each section explains what the form is used for and provides the appropriate
resources and reporting form.
Report a Campus Incident
Use the Campus Incident Form to report issues related to campus safety, accidents, facilities, or other concerns that may affect the safety and well-being of the campus community.

How to Report Academic Misconduct
Use the Academic Misconduct Referral Form to report concerns related to grades, academic dishonesty, quality of instruction, or violations of class or program policies.

How to Report Non-Academic Student Misconduct
Use the Student Misconduct Referral Form to report concerns involving violations of Âé¶¹Ô´´'s non-academic misconduct standards as defined in the Student Handbook.

The CARE Team helps connect students who may be at risk or experiencing challenges with campus and community resources. By reporting a concern, you can help the CARE Team reach out to a student, provide support, and connect them with resources that may help.
