In order to qualify you must:
1. Be a resident of Arkansas, and
2. Have a child(ren) under the age of 21, and

- Federal Tax Return - form 1040 only (Most Recent Year)
- Applicant™s Photo ID (Driver's License or State-issued ID)
- Social Security Card “ Applicant and Child(ren)™s
- Child(ren)™s Birth Certificate
- Proof of DHS benefits noting approval of services within the last 30 days. Accepted
documents include:
- Access Arkansas printout or DHS Benefits Verification Worksheet
